Pregabalin is used to treat pain caused by nerve damage in people with diabetes
What is it?
Pregabalin is a structural analogue of the neurotransmitter GABA. It has analgesic, anticonvulsant, anxiolytic and sleep-modulating activities and is indicated for treatment of epilepsy and neuropathic pain. It has a similar pharmacological action to that of gabapentin. However, it should be noted that gabapentin is not PBS reimbursed for neuropathic pain.
Drug class and mechanism :
This drug is an oral medication that is chemically related to gabapentin (Neurontin, Gabarone). It is used for treating pain caused by neurologic diseases such as postherpetic neuralgiaas well as seizures. It also is used for treating fibromyalgia. The mechanism of action of pregabalin is unknown. Pregabalin binds to calcium channels on nerves and may modify the release of neurotransmitters (chemicals that nerves use to communicate with each other). Reducing communication between nerves may contribute to pregabalin’s effect on pain and seizures.
Who is it for?
Pregabalin is a treatment option for people with refractory neuropathic pain who have not responded to other drugs. Use with caution in people with renal impairment, as pregabalin is renally excreted.
Safety issues :
This drug is generally well tolerated and is associated with dose-dependent adverse events that are mild to moderate and usually transient. As pregabalin alters neurotransmission, it can cause a variety of neurological adverse events. Common adverse effects reported in at least 3% of all patients treated with pregabalin include :
• dizziness
• drowsiness
• blurred vision
• fatigue
• weight gain
• dry mouth
• headache
• impaired balance
• peripheral oedema.
Dosing :
This drug may be taken with or without food. The initial dose for neuropathic pain associated with diabetic peripheral neuropathy is 50 mg three times a day (150 mg/day). The dose may be increased to a maximum dose of 100 mg 3 times daily (300 mg/day) after one week.
